Liposuction is one of the most widely performed aesthetic procedures worldwide, used to reshape body contours and treat localized fat deposits. Despite its widespread use, a thorough understanding of complications, long-term outcomes and the indications for revision surgery is essential to ensure patient safety and satisfaction. This systematic review summarizes PubMed-indexed evidence based on systematic reviews and meta-analyses, focusing on clinical indications, complication rates, outcomes and risk factors associated with both primary liposuction and revision procedures.

How the evidence was selected

For this review, a targeted PubMed search was conducted, including only systematic reviews and meta-analyses published up to 2025. Studies reporting data on complications of aesthetic liposuction, postoperative outcomes and procedure-related risk factors were included. Priority was given to studies providing pooled quantitative data, comparisons between different liposuction techniques and analyses based on large patient cohorts, in order to provide a reliable overview of clinical outcomes and the risks associated with both primary and revision procedures.

Clinical indications and criteria for revision liposuction

Liposuction is primarily indicated for the treatment of localized fat deposits in various body regions, with the aim of improving body contour and correcting contour irregularities. However, revision surgery may become necessary in certain cases, particularly when the initial outcome is unsatisfactory because of asymmetry, surface irregularities, recurrent fat accumulation or late complications that do not respond to conservative management.

Systematic evidence indicates that, although liposuction is generally safe, a significant proportion of patients may require corrective surgery to optimize aesthetic outcomes and achieve a more uniform and satisfactory result.

Incidence and risk profile of complications in primary liposuction

Meta-analytic evidence indicates that liposuction has a low overall complication rate, although several adverse events have been consistently reported. A review involving more than 29,000 patients estimated an overall complication rate of approximately 2.6%, with contour deformities representing the most frequent complication (approximately 2.35%). Other reported complications include hyperpigmentation (1.49%), seroma (0.65%), hematoma (0.27%) and skin necrosis, which remains a very rare event.

These findings confirm that liposuction is generally safe, while also highlighting the importance of early recognition and appropriate management of adverse events to prevent the need for revision surgery and optimize aesthetic outcomes for patients.

Meta-analytic evidence indicates that several factors may influence the risk of complications and, consequently, the need for revision surgery.

– Liposuction technique: Certain approaches, such as laser-assisted or radiofrequency-assisted liposuction, may be associated with slightly higher rates of local complications, including skin irritation or surface irregularities, compared with conventional liposuction.

– Volume of fat aspirated: Procedures involving the removal of large volumes of fat show a slightly increased risk of local and systemic adverse events, although the available evidence remains heterogeneous.

– Patient characteristics: Comorbidities, obesity and deep fat deposits increase the likelihood of significant complications and less satisfactory aesthetic outcomes.

Careful preoperative assessment of these factors is essential to minimize the risk of complications and reduce the likelihood of unplanned revision procedures, thereby optimizing both aesthetic outcomes and patient safety.

Long-term outcomes and factors influencing post-liposuction satisfaction

Although liposuction generally has a low complication profile, individual variability in fat resorption, skin contour definition and patient satisfaction may influence the need for additional procedures.

Revision surgery is often required when aesthetic outcomes do not meet patient expectations or when local complications persist. Meta-analytic evidence shows that overall patient satisfaction is generally high; however, the standardization of reported outcomes remains limited by the heterogeneity of the definitions of “success” and “failure” used across different studies.

Careful follow-up and individualized patient management are essential to optimize long-term aesthetic outcomes and reduce the need for unplanned revision procedures.

Systematic reviews indicate that the prevention of complications, and consequently the need for revision surgery, relies on a comprehensive and carefully planned approach. The main strategies include:

– Comprehensive clinical assessment: Detailed evaluation of the patient and their aesthetic expectations to identify suitable candidates and reduce the risk of postoperative dissatisfaction.

– Technical standardization: Application of standardized aspiration techniques adapted to individual anatomical characteristics.

– Rigorous postoperative follow-up: Early monitoring to detect contour irregularities, seroma formation or other local complications.

The adoption of a multidisciplinary approach and well-defined surgical protocols helps minimize the need for unplanned revision procedures while improving overall aesthetic outcomes, patient safety and satisfaction.
